CAN I GET A SAME-DAY APPOINTMENT? DO YOU OFFER WALK-IN CLINIC SERVICES?
It is our desire to meet your medical needs in a timely manner. Sometimes we are able to accommodate same day appointments. It is best to call the location of your choice and check availability. Sometimes if we are booked at one location, we can still see you at another.
WHAT IS YOUR NEW PATIENT PROCEDURE?
Please call for an appointment and let the receptionist know that you are a new patient. Please arrive 20-30 minutes before your scheduled appointment in order to fill out new patient paperwork and for us to enter the information into our system. Please bring any applicable insurance card(s) and all medications that you are currently taking. If you have medical records from previous clinics/hospitals, please bring them with you as well.

How it Works
PCHS provides a sliding fee discount program for services based on income and number of family members. The sliding fee discount is confidential and persons applying for the discount must provide documentation of income, such as employee paycheck stubs, government benefits, etc. Depending on qualifications, patients may be eligible for a 100%, 75%, 50%, or 25% discount on services.
Who can Qualify
Patients may qualify for the sliding scale fee dependent on family size and income. The sliding scale fee is based on the Federal Registered poverty guidelines. Guidelines are updated each year, but typically uninsured patients who fall below 200% of the Federal Poverty Limit are eligible for a discount. Insured patients with a high deductible and who meet the requirements may apply as well.
How to Apply
At new patient registration, patients have the option to apply for the sliding scale fee. Patients will be asked to fill out a form, as well as provide proof of income for all members of the household, proof of address, and a picture ID. Other necessary documents may include: current regular payroll stub, income tax return, and/or utility bill.
